1. Introduction

This Consent Form authorizes CareNestPro Ltd and its approved partners to conduct background verification and medical screening as part of the CareNestPro Connect onboarding process. These checks are essential to ensure safety, trust, and compliance on the Platform.

2. Personal Information

By signing or submitting this form through the app, you confirm that the information you provide (name, address, date of birth, contact, and ID) is accurate and may be used for verification purposes.

3. Scope of Background Check

You consent to the collection and verification of the following: Identity Verification (NIN, BVN, passport, or valid national ID); Criminal Record Check by Background Check International (BCI); Employment History & Reference Verification; Educational Qualification Confirmation; and Address Validation. These checks are used solely to assess suitability for care assignments.

4. Medical Screening Consent

You authorize CareNestPro Ltd and its approved medical partners to conduct medical screenings such as Hepatitis B, HIV 1 & 2, TB (Mantoux), Drug of Abuse screening, and Pregnancy tests (for females). Results are confidential and may affect eligibility to operate as a care provider.

5. Data Privacy

All data will be processed and stored securely under the Nigeria Data Protection Regulation (NDPR). CareNestPro Ltd will not disclose or sell your information to third parties except for verification, medical, or regulatory purposes.

6. Rights of the Applicant

You have the right to request access to your verification results, correct inaccurate information, and withdraw consent at any time (withdrawal may affect eligibility to use the Platform).

7. Duration of Consent

This consent remains valid for the duration of your registration and engagement with CareNestPro Connect, unless withdrawn in writing.

8. Declaration

By signing or submitting this consent electronically, you confirm that you understand and voluntarily authorize CareNestPro Ltd and its partners to perform these verifications. You release CareNestPro Ltd, its agents, and partners from liability arising from lawful disclosure of this information.
